Position Description

Under the supervision Project Director for the CCBHC the CCBHC Community Health Worker Coordinate and provide access for patients/caregivers to community and individual support services. Meets regularly with the care team, acting as a liaison and advocate for the patient/caregiver, to discuss cases and formulate care plans and document created plans within the Electronic Health Record. Provides education on health conditions and strategies to implement identified care plan goals including both clinical and non-clinical needs.

Essential Functions

  1. Provides education on health conditions and strategies to implement identified care plan goals including both clinical and non-clinical needs.
  2. Identifies community resources (i.e. social services, workshops, etc.) and educates the patients/caregivers on how to utilize and access them as appropriate.
  3. Tracks referrals provided to patients/caregivers and documents outcomes within the Electronic Health Record.
  4. Reviews transitions of care reports provided through the Health Information Exchange and arranges post-discharge follow-up.
  5. Coordinates and provides access to chronic disease management including self-management support and resources.
  6. Implements wellness and prevention initiatives.
  7. Facilitates health education groups.
  8. Assists with increased new patient and repeat care visits by promoting the center(s) within the community and connecting individuals to potential referral sources such as community-based organizations, hospitals/urgent cares, and other groups and/or programs providing assistance to the under serviced patient population.

What are we looking for?

  • High School Diploma required.
  • Current MiCHWA Community Health Worker certification preferred.
  • Bilingual (Spanish/English) preferred.
  • Experience in human services, social work, and/or public health preferred.
  • A minimum of one year of experience working in a clinical environment required or equivalent experience.
  • Experience working for a community health center or Federally Qualified Heath Center preferred
